Remove port equal functions
* doc/ref/api-io.texi (Port Implementation): Remove mention of port
equal functions.
* NEWS: Update.
* libguile/ports.c (scm_set_port_equalp): Remove.
* libguile/ports.h (scm_t_ptob_descriptor): Remove equalp function.
